In the event of my child suffering an injury during a club activity, I give my consent for first aid to be administered.

Photo/Video Consent

At times the club may wish to take photos or videos of teams or individuals. We adhere to FAI guidelines to ensure these are safe and respectful and used solely for the purposes for which they are intended, which is for promotions and celebration of the activities of the club and for training. I give my consent for my child(rens) image to be used for this purpose.
